I declare that the accident history I have provided is true and correct. I acknowledge that if this information is found to be false or intentionally withheld, Veloce reserves the right to cancel my rental, forfeit my deposit, and/or seek civil damages for any losses incurred.

I understand that if I do, I may forfeit my deposit.

I consent to Veloce possibly performing background checks and acknowledge that if this is found to be false, Veloce may cancel my rental and I may forfeit my rental payment and/or deposit.

I acknowledge that a breach may result in civil action by Veloce and criminal charges by the state.
